Pathway #2: Securing Health Care Coverage


The Securing Health Care Coverage Pathway works to find health insurance for children of families with low to moderate incomes. A Community Health Worker helps each client find the most appropriate insurance option, complete application packets, and necessary documents to ensure a successful insurance coverage outcome.

This pathway is considered successful when a client has obtained health care coverage for their child.
