ACCEL Care Pathway

Pathway #3: Obtaining a Medical Home


This ACCEL Care Pathway helps families obtain a medical home. Families who would otherwise use the hospital emergency room are assisted in finding a regular source of primary care for their child.

Local hospital emergency room staff refer families to ACCEL when the child lacks insurance. Community Health Workers assess the families for health care coverage and match them with clinicians based on client location, insurance type and preference. The client is also provided health education services and assistance working through any barriers that might inhibit them from successfully establishing a medical home.
