Pathway #4: Utilizing a Medical Home


Maneuvering through newborn checks, immunizations, and common illnesses can be daunting for parents. The ACCEL Care Pathway strives to come alongside parents of newborns on Medi-Cal, and assist them in a successful first year of life for their child.

For eight months to one year, a Community Health Worker assists in: establishing a regular source of primary care for the newborn, providing health education to the parents, and overcoming barriers that may inhibit the parents from accessing appropriate medical care for their child. Well baby checks and immunization schedules are monitored by the Community Health Worker, who reminds the parents of appointments and problem-solves with the parents when appointments are missed.
