Low-Cost Health Plans and Eligibility Requirements.*

Medi-Cal

Who qualifies for Medi-Cal? Children birth through 18 in families with no or low income (family income at or below 200% of the Federal Poverty Level), pregnant women and infants. Must be a California resident.

What's covered? Includes health, dental, vision, and behavioral health. Includes hospitalization. Includes pregnancy related care (prenatal and delivery). Mothers are covered up to 60 days after delivery.

Cost? There is no cost for qualifying individuals.

Healthy Families

Who qualifies for Healthy Families? Children birth through 18 in families with moderate income (family income at or below 250% of the Federal Poverty Level). Must be a California resident. Must be ineligible for no cost Medi-Cal or employer based coverage.

What's covered? Offers a variety of health, dental and behavioral health plans from which to choose.Includes hospitalization.

Cost? $4-15 per child, per month with a $45 family maximum per month. $5 co-pay for doctor visits and prescriptions.;

Kaiser Child Health Plan

Who qualifies for Kaiser Child Health Plan? Children birth through 18 in families with moderate income (family income at or below 300% of the Federal Poverty Level). Must have a California address and live within 30 miles of a Kaiser facility. Must be ineligible for no-cost Medi-Cal, Healthy Families or employer-based coverage.

What's covered? Includes health, dental, vision and behavioral health.Includes hospitalization. Must be seen at a Kaiser facility.

Cost? $8-15 per child, per month with a $45 family maximum per month. $5-10 co-pay for doctor visits and prescriptions.

California Kids

Who qualifies for CaliforniaKids? Children age 2-18 in families with moderate income (family income at or below 300% of the Federal Poverty Level). Must have a California address. Must be ineligible for no-cost Medi-Cal, Healthy Families or employer-based coverage.

What's covered? Health offered through Blue Cross. Dental and vision offered through Safeguard. Behavioral health offered through the Holman Group. Does NOT include hospitalization.

Cost? $15 per child per month, with a $45 family maximum per month.Small co-pay (generally $5 for some services).

Healthy Kids, Healthy Future

Who qualifies for Healthy Kids, Healthy Future? Children birth through 18 in families with moderate income (family income at or below 300% of the Federal Poverty Level). Must have a California address. Must be ineligible for no-cost Medi-Cal, Healthy Families, Kaiser Child Health Plan, CaliforniaKids or employer-based coverage.

What's covered? Includes health, dental and vision. Includes hospitalization.

Cost? $15 per child per month, with a $45 family maximum per month. Small co-pay.

AIM

Who qualifies for AIM? Pregnant women and infants in moderate income families (income between 200-300% of Federal Poverty Level). Must be a California resident for at least six months.

What's covered? Comprehensive medical care provided for mother (not just maternity).Mother continues to be covered up to 60 days after delivery.

Cost? 1.5% of annual income.

CHDP

Who qualifies for the Child Health and Disability Prevention Program (CHDP)? Children birth to 21with full scope and share of cost Medi-Cal. Uninsured children birth to 18 with family income below 200% of the Federal Poverty Level. Must have a California address.

What's covered? Preventive health check-ups and education based on a child's age.

Cost? No charge for visit (periodocity table used to determine frequency of visits allowed).

CCS

Who qualifies for California Children Services (CCS)? Children under age 21 with serious medical and disability conditions covered by CCS. Must be a California resident with family income less than $40K. Families with income over $40K may qualify if out-of-pocket expenses exceed 20% of income.&Family income NOT a factor for children in no-cost Medi-Cal or Healthy Families.Children may have other insurance while covered by CCS.

What's covered? Diagnosis and treatment of CCS eligible conditions. Includes hospitalization and in-home nursing care. Includes occupational and physical therapy services.

Cost? One time enrollment fee based on income. Annual assessment fee of $20. No monthly premium.
