The future of health care for all will depend on the intelligent use of health information technology (IT) to improve access and quality. Consistent with these views, ACCEL has made important strides in promoting and integrating health IT in the county.

Centralized Patient Registry and Pathway Management Application: With a grant from the Agency for Healthcare Research and Quality (AHRQ), ACCEL utilizes a shared web-based application to monitor ACCEL patients through a Care Pathway. The application allows ACCEL agencies in multiple locations to track, share, and update patient progress more efficiently. The use of technology has allowed for real-time, efficient communication across agencies and standardized methods for patient-system navigation. All ACCEL agencies are utilizing the web-based application and users are provided with formal one-on-one or group training.

Telehealth: With seed funding from the California HealthCare Foundation, ACCEL designed and implemented a pain management telehealth program with the University of California, Davis Medical center in 2010. The use of telehealth technologies are also being used to provide countywide continuing medical education opportunities for providers. ACCEL continues to seek new and innovative opportunities to use telehealth tools, including remote patient monitoring applications, to increase access to care and build local provider capacity to deliver high quality services to their patients.
